APPLICATION FORM FOR THE PROTECTION OF PERSONAL DATA

In accordance with the Law No 6698 on Protection of Personal Data (“Law”) and relevant legislation, you are granted with the right to information by applying to the data controller, namely Best Dental İstanbul Ağız ve Diş Sağlığı Hizmetleri Ltd. Şti (“Best Dental İstanbul”).

  1. PURPOSE With this application form, you have the following rights:
  2. a) to learn whether your personal data are processed or not,
  3. b) If they are processed, to request information relating thereto,
  4. c) to learn the purpose of the processing of my personal data and whether they are used for the intended purposes or not,
  5. d) to learn third parties resident at home or abroad to whom your personal data are transferred,
  6. e) If they are processed incompletely or wrongly, to request correction of them,
  7. f) to request deletion/destruction thereof under the conditions set out in Article 7 of the Law,
  8. g) to request that the third parties to whom your personal data have previously been transferred are informed about the transactions set out in paragraphs (e) and (f),
  9. h) to raise objections to any conclusions which may arise against your interests upon analysis of your processed personal data solely by means of automatic systems,
  10. i) to request compensation for your damages due to the unlawful proce
  1. THE APPLICATION PROCEDURE shall be finalized by our organization free of charge within the shortest time possible depending on the nature of the request and within thirty days at the latest. However, in the event the procedure requires any additional costs, an extra fee may be charged. Our organization may accept the request and process it, or may reject it in accordance with the written procedure together with its reasoning. In the event that your application is caused by a fault of our organization, the fee shall be refunded to the relevant person.

III. CONTENT OF THE APPLICATION the information provided to you must be correct and updated, our organization does not assume any responsibility for the request to be caused by the incorrect or incomplete information provided in the content of the form or unauthorized application. Our organization reserves the right to request additional information and document (eg.: a copy of ID card or driver’s license, etc.) for identification purposes in order to ensure data security.

  1. IV. EXCEPTIONS On the other hand, due to the fact that the following conditions are not stipulated by the law, the right to submit a request for these issues is not granted:
  2. a) Personal data is processed by natural persons within the scope of purely personal activities of the data subject or of family members living together with him in the same dwelling provided that it is not to be disclosed to third parties and the obligations about data security is to be complied with.
  3. b) Personal data is processed for the purpose of official statistics and for research, planning and statistical purposes after having been anonym
  4. c) Personal data is processed with artistic, historical, literary or scientific purposes, or within the scope of freedom of expression provided that national defense, national security, public security, public

order, economic security, right to privacy or personal rights are not violated or they are processed so as not to constitute a crime.

  1. d) Personal data is processed within the scope of preventive, protective and intelligence activities carried out by public institutions and organizations duly authorized and assigned to maintain national defense, national security, public security, public order or economic security.
  2. e) Personal data is processed by judicial authorities or execution authorities with regard to investigation, prosecution, criminal proceedings or execution In the following cases, only the right to demand compensation for the incurred damage is granted a) Personal data processing is required for the prevention of a crime or crime investigation. b) Personal data processing is carried out on the data which is made public by the data subject himself. c) Personal data processing is required for inspection or regulatory duties and disciplinary investigation and prosecution to be carried out by the public institutions and organizations and by professional associations having the status of a public institution, assigned and authorized for such actions, in accordance with the power conferred on them by the law, d) Personal data processing is required for the protection of State’s economic and financial interests with regard to budgetary, tax-related and financial issues.
